If you’re in stages 1, 2, or 3, your protein intake may be limited to 12 to15 percent of your calorie intake each day. This is the same level recommended by the Dietary Reference Intakes (DRIs) for a healthy diet for normal adults. If you’re in stage 4 of CKD, your dietitian may advise you to reduce protein to 10 percent of daily calorie intake.

Third, glomerular sclerosis is caused by intraglomerular hypertension (induced by renal vasodilatation or from ischemic injury induced by hyaline narrowing of the vessels supplying the glomeruli). These different histologic patterns appear to have similar prognostic significance.

Kidney damage in type 1 diabetes is the largest cause of chronic kidney disease in the working age group.

Kidney disease in people with type 2 diabetes is increasing because of the increasing prevalence of people with diabetes, improved cardiovascular survival and the trend to younger onset of type 2 diabetes.

The prevalence of microalbuminuria in patients with type 1 diabetes at 30 years' disease duration is approximately 40%.

The prevalence of microalbuminuria in patients with type 2 diabetes at 10 years' disease duration is approximately 20-25%.

Stages of Chronic Kidney Disease (CKD)

In the first stage, the GFR is equal to or greater than 90 mL/min, which is same as that of a healthy kidney. There are almost no symptoms and hence, diagnosing the condition is difficult. But it is generally recommended for patients suffering from diabetes, to undergo tests, which can give details of the amount of creatinine or urea in the blood. An increased amount of creatinine in the blood or proteins in the urine is a clear indication that a person is suffering from chronic kidney disease. Other methods used to understand the state of a person's kidney are MRI, ultrasound, X-ray and CT scan. If the problem is diagnosed at this stage, the treatment becomes relatively simpler and medication can be used to stop, retard or reverse the CKD.

The symptoms in this stage are not identifiable, just as in the first stage. The GFR lies between 60 and 89mL/min. The diagnosis can again, be done through X-ray, MRI, ultrasound and CT-scan. Kidney dialysis is generally not required if a person is detected with kidney disease at the second stage.

This is the stage where the symptoms start showing. The GFR falls somewhere in between 30 to 59 mL/min. The patient shows symptoms of fatigue and breath shortage. Liquids start accumulating in different parts of the body and this is visible due to the swelling of the hands and legs. Urine color of the person also shows identifiable changes. It changes to dark orange, red or brown. A person, if diagnosed at the third stage, is referred to a nephrologist, who performs various lab tests to understand the root cause of the problem and suggests the type of treatment. A person in this stage, should also consult a dietitian who would recommend a diet that best suits his condition. If a patient is suffering from polycystic kidney disease, he may experience pain at the back of his body, around the area where the kidneys are located.

With the GFR falling further, the patient starts showing additional symptoms. With increase in urea content in the patient's blood, he normally develops a bad breath. Nausea and loss of appetite are the common symptoms of a patient going through the fourth stage of CKD. There are a variety of nerve problems and he loses his capability to concentrate. The GFR in the fourth stage, is between 15 to 29 mL/min. Hemodialysis and peritoneal dialysis are the common treatments at this stage. A kidney transplant is also recommended by many nephrologists, if the GFR is very close to 15 mL/min.

Eat a Healthy Diet for CKD

We literally are what we eat—we build our cells out of our food. So, if you eat mostly fast food and drink sugary sodas, you may be able to improve your kidney health by eating better foods. Tight control of blood sugar can help slow the progression of kidney disease. Your HbA1c levels, which measure your blood sugar control over a period of 3 months, should be less than 6.5%.

Quit Smoking

In people with CKD, smoking is linked to an increase in the amount of protein spilled in the urine. In smokers with diabetes, CKD may progress twice as fast. Scientists are not sure why this is the case, but if you have kidney disease and you smoke, quitting may help slow down the damage.

Avoid Certain Pain Medications

Some over-the-counter pain pills containing ibuprofen or naproxen, and even acetaminophen (e.g., Motrin®, Advil®, Aleve®, Tylenol®) can affect kidney function. This is especially true if you have kidney, heart, or liver disease or you take diuretics (water pills). Avoid using combinations of these pain pills and caffeine—both at once can further damage your kidneys.

Phosphorus Pyramid for CKD Provides Diet Advice

It's a Cyclical Ketogenic Diet, or CKD for short. CKD basically means that you cycle periods of low carb, high protein, and high fat with periods of high carb, high protein, and low fat. Phosphorus is a concern at all stages of CKD, the researchers noted. The pyramid can help both CKD and dialysis patients understand what to eat and what to avoid, with appropriate tailoring. Here are some basic nutrition strategies:

In non-dialysis patients, consider restricting protein intake. (Those on dialysis require extra protein.) In a mixed diet, each gram of protein is usually accompanied by 12–14 mg of phosphorus.

Shift from phosphorus-rich foods to low phosphorus foods. Take into consideration bioavailability. Plant foods contain phosphorus but less than half of the mineral content is absorbed by the body. The phosphoric acid in soft drinks, by comparison, is almost completely absorbed.

Boil foods to reduce their mineral content, including phosphorus (then discard the water). According to one study, boiling reduces phosphorus by 51% for vegetables, 48% for legumes, and 38% for meats.

Try to identify and avoid phosphate additives. Processed foods contain considerable amounts of added phosphorus, including from preservatives.

The Phosphorus Pyramid

The food pyramid color codes food items similar to a traffic light: greens for go, yellows for slow, and reds for stop.

It consists of 6 levels with foods arranged by their phosphorus content, phosphorus to protein ratio (no more than 12 mg/g is favorable) and phosphorus bioavailability. Here's an overview of each level from 1 (low phosphorus) to 6 (high phosphorus):

Green: Foods with a very low phosphorus content include protein-free foods, fruit, vegetables, egg white, olive oil, and sugar. These items are not limited, unless a patient is overweight, diabetic, or receiving dialysis.

Light green: Foods with phytate, a less absorbable form of phosphorus, include cereal, rice, pasta, white bread, and legumes. 2–3 servings per day.

Yellow: Among meats, choose lamb, rabbit, ham, or fish (e.g., trout, tuna, cod, hake, and sole). Avoid farmed fish because it has been fed with phosphorus to promote growth. Also choose milk and yogurt. No more than 1 serving per day.

Orange: Foods with a higher phosphorus to protein ratio include turkey, offal, shrimp, squid, salmon, and soft cheeses. No more than 1 serving per week.

Orange-Red: Foods with a very high phosphorus content include nuts, egg yolk, and hard cheeses. No more than 2-3 servings per month.

Red: Processed foods with phosphorus-containing additives include colas, processed meat, and processed cheese. Avoid as much as possible.

Gout Prevalent in German CKD Cohort

Gout is fairly common among chronic kidney disease (CKD) patients, a recent analysis of the German Chronic Kidney Disease study suggests.
Investigators evaluated data from more than 5,000 CKD patients and identified gout in 24.3%, according to results published in Nephrology Dialysis Transplantation. As kidney function decreased, gout prevalence increased, from 16% among patients with eGFR above 60 mL/min/1.73 m2 (with proteinuria) to 36% among those with eGFR below 30.

Roughly a third of patients with gout were not taking any medication for treatment at the time of the analysis. In addition, half of gout patients on urate-lowering therapy still showed signs of hyperuricemia. “Pharmacological management of gout in patients with CKD is suboptimal,” the investigators suggested.

The following factors were linked with gout: serum urate, lower eGFR, older age, male sex, higher BMI and waist-to-hip ratio, higher triglyceride and C-reactive protein (CRP) concentrations, alcohol intake, and diuretics use.

The researchers hope to assess whether gout and hyperuricemia increase the risk of CKD progression and cardiovascular events with future follow-up of this cohort.

Lower CKD Risk Among Fit Veterans

As aerobic fitness increases, the risk of chronic kidney disease (CKD) progressively decreases suggesting a protective role for exercise, according to a new study of male veterans.

“The findings of the present study demonstrate that health benefits associated with higher exercise capacity also extend to lowering the risk of developing CKD,” stated Peter Kokkinos, PhD, of Veterans Affairs Medical Center in Washington DC, and colleagues.

The investigators prospectively assessed peak exercise capacity for 5,812 male veterans at the center using a standard treadmill test. The men were referred for exercise testing for clinical reasons.

All participants were free of CKD stage 3 and above at baseline with an estimated glomerular filtration rate of 60 mL/min per 1.73 m2 or higher. CKD developed in 1,010 men (17%) during follow-up.

According to results published in Mayo Clinic Proceedings, the risk of CKD declined by 22% with every 1 metabolic equivalent (MET) increase in exercise capacity. Compared to the least fit individuals, the odds of CKD were 13%, 45%, and 58% lower in the low-, moderately-, and high-fit groups, respectively.

Researchers adjusted the fitness groupings for age. They also adjusted their models for CKD predictors, such as race, diabetes, hypertension, dyslipidemia, alcoholism, glucose-lowering medication, and lipid-lowering medication.

Men with lower GFR may be deficient in vascular density and recruitment, the researchers suggested.  Also, a significant number of the veterans had a high body mass index and diabetes. Insulin resistance, dyslipidemia, and hypertension may contribute to vascular stiffness and systemic inflammation, setting the stage for CKD.

By contrast, physically active men have greater capillary recruitment during exercise, which likely corresponds to a greater renal reserve and a lower likelihood of CKD.

Starting an aerobic exercise routine is reasonable for many men with their doctors' permission, the investigators noted: “The average exercise capacity of approximately more than 6.5 METs necessary to realize these health benefits is achievable by many middle-aged and older men by daily exercises, such as brisk walking.”

Screen for CKD Among People of Low Socioeconomic Status

In many CKD patients, previous renal disease or other underlying diseases are already known. Screening those who have neither symptoms nor risk factors for CKD is not recommended. The prognosis of patients with chronic kidney disease is guarded as epidemiological data have shown that all cause mortality increases as kidney function decreases.

Screening people of low socioeconomic status for chronic kidney disease (CKD) may prove more valuable than screening seniors, according to Dutch researchers. The study found that people of low socioeconomic status are at higher risk of CKD and its complications, including kidney decline and cardiovascular events, compared to adults older than age 60. For the study, the investigators compared 3 screening approaches in 3,411 individuals:

Traditional CKD screening of people with diabetes, high blood pressure, or a history of cardiovascular disease

Traditional screening with the addition of seniors

Traditional screening with the addition of people of low socioeconomic status (i.e., only a primary school education)

Participants underwent 4 rounds of screening over more than 9 years. The numbers of individuals needed to screen to detect one CKD case were 5.6 in Approach 1 and 6.5 each in Approach 2 and 3. In Approach 2 (seniors), the odds of cardiovascular events were 87 to 92% higher among persons with CKD compared to those without the disease. The odds were more striking in Approach 3 (low income individuals), however: 128 to 231%.

CKD is a Gout Risk Factor

Chronic kidney disease is identified by a blood test for creatinine. Chronic kidney disease (CKD) appears to be an independent risk factor for gout, according to American researchers.  Stage 5 CKD is often called end stage renal disease, end stage renal failure, or end-stage kidney disease, and is synonymous with the now outdated terms chronic kidney failure or chronic renal failure.

For the study, investigators observed the development of gout among participants in the Framingham Heart Study 1948-2002.

Using Cox proportional hazard models, the researchers estimated the odds of CKD leading to gout among men and women separately. They adjusted results for other risk factors, including age, alcohol consumption, smoking history, hypertension, diabetes, and body mass index.

According to results published in BMJ Open, the incidence of gout among CKD patients was 6.82 per 1,000 persons per year compared to 2.43 for those without the disease. In multivariable models, CKD was associated with double the odds of gout among men and women.

The study adds to the growing body of epidemiological research supporting the hypothesis that CKD is a risk factor for future gout.
